Disability Support Worker – PEG Feeding & Personal Care

Triniti Home Care is seeking dedicated and reliable Disability Support Workers to support an NDIS participant with PEG feeding, personal care, daily living activities, and community access.

About the Role
You will provide one-to-one support, assisting the participant with personal care, PEG feeding, daily routines, appointments, and community activities.
The successful applicant must be reliable, patient, and confident following care plans and established procedures. You will be expected to maintain the participant's dignity, safety, comfort, and wellbeing at all times.
Key Responsibilities
- Provide one-to-one disability support in the home and community.
- Provide PEG feeding support in accordance with the participant's care plan and training.
- Assist with personal care, hygiene, dressing, toileting, and other daily living activities.
- Support safe attendance at appointments and community activities.
- Follow the participant's Care Plan, Risk Assessment, and relevant clinical instructions.
- Maintain safe and hygienic practices during PEG feeding and personal care.
- Support the participant's communication, independence, and daily routines.
- Monitor the participant's wellbeing and report any concerns appropriately.
- Complete accurate progress notes and incident reports.
- Work collaboratively with the participant's family, support team, and healthcare professionals.
Essential Requirements
- Minimum 12 months' experience in disability or community support.
- Experience with PEG feeding is highly desirable.
- Current NDIS Worker Screening Check.
- Working With Children Check.
- Current First Aid and CPR Certificate.
- NDIS Worker Orientation Module.
- Current Australian Driver Licence.
- Fully insured and reliable vehicle.
- Good communication and documentation skills.
- Willingness to complete participant-specific PEG feeding training if required.
Highly Desirable Experience
Experience supporting participants with:
- PEG feeding.
- Personal care and complex daily living needs.
- Community access and appointments.
- Medication support.
- Clinical or health-related support needs.
- Participants requiring ongoing one-to-one support.

Shifts Available
- Morning and afternoon support.
- Weekday and weekend shifts.
- Ongoing and consistent shifts available.
