Your opportunity:
To deliver high level social work clinical services and expertise to patients identified for the rapid access geriatrician-led model of care, as a multidisciplinary team member.
This short sub-acute service (up to 14 days) approach aims to reduce prolonged in-hospital patient admissions (extended stays). The position will facilitate a safe discharge and monitoring so that patients can continue living at home while improving their functioning and connecting with care in the community.
This position will focus on preventing inappropriate or avoidable admissions by supporting the needs of the patient, their family, and significant others through sub-acute supports in the home environment, to optimise their psychosocial outcomes and achieve maximum benefits from health care services.
This position reports to the Director of Social Work.

Our offer:
You will receive a competitive remuneration package value up to $39,770 p.a., comprising salary rates $39.87 - $58.60 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP3) (Permanent Part Time, 22.8 hours p.f.).
