Integrated with the St Vincent’s hospital and community care network we are a multidisciplinary team of physicians, allied health, nursing and personal care workers supporting individuals to leave hospital sooner, remain in their homes and achieve their goals. We are an experienced, supportive and dynamic team committed to the growth of quality care within the community.
About the Role
We are seeking an enthusiastic Care Coordinator (Care Partner) with a background as a Registered Nurse or Allied Health Professional and broad experience across Support at Home to join our community aged care team.
This role is part of a growing service focused on supporting older people to live well at home, coordinating person-centred care that aligns with individual goals, preferences and needs. You will play a key role in fostering strong partnerships with clients and families and contributing to services that promote independence, wellbeing and quality of life in the community.
This is a Full Time hybrid position, working a mix of in-office (Eltham) and from home.
On a daily basis you will:

Provide care coordination ensuring that services are provided in accordance with the Support at Home guidelines.

Budget planning and oversight

Care planning

Working closely with schedulers and clinical services team and clients
Your Contribution

Relevant clinical experience in acute and/or community setting

Relevant certification as a Registered Nurse or an Allied Health Professional
