We are seeking a compassionate, organised, and proactive Care Partner Assistant to join our growing team. This role is focused on supporting our Care Partner team with client administration, documentation and coordination, helping ensure the smooth delivery of high-quality, person-centred care. Fluency in Khmer or Serbian is essential for this role.
As our organisation continues to grow, we're expanding our Care Partner team by introducing the Care Partner Assistant role. This position has been designed to provide dedicated support to our Care Partners while creating a structured pathway for individuals looking to build a career in care management.
Role Overview
As a Care Partner Assistant, you'll provide administrative support to multiple Care Partners across the client journey. You'll prepare service documentation, assist with individualised budgets and care plans, coordinate paperwork, communicate with clients, representatives and service providers, and ensure client records remain accurate and compliant.
You'll play an important role in helping our clients receive timely, high-quality care while supporting the Care Partner team to meet organisational, legislative and Support at Home program requirements.

Key Responsibilities
- Provide administrative and coordination support to multiple Care Partners across a shared client portfolio.
- Prepare Service Agreements, variation letters and other client documentation.
- Prepare and update individualised client budgets.
- Assist with updating care plans as directed by the Care Partner.
- Liaise with internal departments, clients, representatives and service providers to support the timely coordination and delivery of services.
- Liaise with contractors and internal departments to obtain service quotes, coordinate services and raise purchase orders.
- Maintain accurate and up-to-date client records and documentation.
- Assist with client communication during periods of leave or increased workload.
- Undertake home visits, either alongside a Care Partner or independently as directed, to support administrative tasks, client documentation and routine client communication.
- Ensure all work is completed in accordance with organisational policies, procedures and Support at Home program requirements.
- Raise and document incidents, hazards and client feedback within organisational systems, ensuring timely escalation where required.

Requirements
- Ability to speak Khmer or Serbian
- Highly organised with excellent attention to detail.
- Confident using Microsoft Office and client management systems.
- Able to manage competing priorities and meet deadlines.
- A strong communicator with excellent written and verbal communication skills.
- Professional, empathetic and customer-focused.
- Comfortable working both independently and as part of a collaborative team.
- An interest in building a long-term career in aged care, with a desire to progress into a Care Partner role over time.
- Current Victorian Driver Licence
- Willingness to travel across the service area
- Current National Police Check (less than 3 months old) or NDIS Worker Screening Check
- Current First Aid and CPR Certificate
Desirable Experience
- Previous experience in aged care, healthcare, disability or community services.
- Knowledge of the Support at Home Program.

Pay: $60,000.00 – $65,000.00 per year
