About the Role
The Aboriginal Chronic Care team provides culturally appropriate and effective support to Aboriginal and Torres Strait Islander people living in the community over the age of 15 years.
Aboriginal Chronic Care aims to enhance management of chronic conditions through close collaboration with the person’s General Practitioner and access to care coordination, cultural support, specialist services and provision of supplementary services.
The Clinical Nurse Consultant (CNC) provides expert clinical advice to patients, carers and other health care professionals within a defined specialty through the delivery of specialist consultant service which:
- Develops implements and evaluates care management plans for patients with complex health needs.
- Provides leadership in the ongoing development of clinical practice.
- Initiates and utilises findings of research in the provision of clinical services.
- Contributes to the development and delivery of specialty related education programs.
- Participates in formal processes for the strategic and operational planning for the clinical service.
The Aboriginal Chronic Care team is based across Royal Prince Alfred Hospital, Redfern Health Centre and Canterbury Hospital.

Ideal Candidate
- Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field.
- Holds approved postgraduate qualifications relevant to the specialty or such other qualifications or experience deemed appropriate.
- Demonstrated leadership skills and excellent interpersonal, oral and written communication skills.
