Key Responsibilities:

Build strong, trust-based relationships with clients and families

Lead client onboarding, reviews, and ongoing care coordination through a wellness and reablement lens

Develop and formalise individualised care plans and service agreements

Actively promote Dovida’s services to ensure high-quality support and stronger business outcomes

Provide ongoing feedback and collaboration with our Caregiver team

Document interactions and keep all stakeholders informed
About you
-
Previous experience in aged care and aged care systems, ideally Home Care Packages/Support at Home
-
Previous experience in case management is desirable
-
Outstanding relationship-building and communication skills
-
A confident, professional approach to customer engagement and delivering on promises

Experience with Microsoft Office Suite – especially Outlook, Excel, and Word
-
Proactive, solutions focused and engaging
-
Valid Australian Driver’s Licence and willingness to travel across the region
