Disability and Healthcare Support Pty Ltd (trading as Support Foundation) is an established NDIS-registered provider delivering disability support services across Sydney. The organisation provides a range of services including supported independent living (SIL), community participation, and in-home care, supporting participants with diverse and complex needs.
About the Role
We are seeking multiple dedicated and experienced Personal Care Assistants to support NDIS participants in achieving independence, wellbeing, and quality of life. The role involves providing person-centred care to individuals with physical, intellectual, and psychosocial disabilities in both home and community settings.
Key Responsibilities
· Providing personal care to clients including assistance with showering, dressing, grooming, toileting, and eating
· Supporting clients with mobility, transfers, and use of assistive equipment
· Assisting with meal preparation and feeding where required
· Following individualised care plans and supporting clients with daily living activities
· Supporting clients with community access, appointments, and social participation
· Observing and reporting any changes in clients’ condition or wellbeing to supervisors
· Maintaining accurate records including progress notes, care documentation, and incident reports
· Communicating effectively with clients, families, and care team members to ensure continuity of care
Skills and Experience Required
· AQF Certificate II or III qualification in Individual Support, Disability, Aged Care, or related field,
· Minimum 1 year of experience in disability or community care
· Demonstrated experience supporting clients with complex needs (highly regarded)
· Strong understanding of person-centred care and NDIS practices
· Excellent communication and interpersonal skills
· Ability to work independently and as part of a team
Mandatory Requirements
· NDIS Worker Screening Check
· Working With Children Check (if applicable)
· First Aid and CPR certification
· Valid driver licence and access to a vehicle
· Full working rights in Australia
