The Role
We are seeking an experienced Care Partner, you’ll be the link between clients, families, and support workers. Ensuring care is delivered with empathy, accuracy, and heart. This is a dynamic role where you’ll combine client engagement, care coordination, and rostering responsibilities, making every day varied and rewarding.
Key Responsibilities

Support clients in navigating their Support at Home funding.
- Build strong, trusting relationships with clients, families, and support staff.
- Coordinate services to match each client’s needs, goals, and preferences.
- Create and manage effective rosters for our team of support workers.
- Respond promptly and empathetically to client enquiries and changing circumstances.
- Collaborate with internal teams and external providers for seamless service delivery.
- Maintain accurate documentation and uphold compliance with sector standards.
About You

Experience in care coordination, rostering, or client services.
- Knowledge of the NDIS and/or Aged Care sector (especially self-managed NDIS & HCP).
- Excellent communication and time management skills.
- A proactive, can-do attitude with a passion for client care.
- Confident computer skills (rostering systems or CRMs a bonus).
- Police Check, WWCC, NDIS Worker Screening Check (or willingness to obtain).
- Current First Aid & CPR (or willingness to obtain).
- Minimum of two doses of COVID-19 vaccination.
- Comfortable in fast-paced, regulated, or changing environments
- Proactive and committed to continuous improvement
