We are seeking a compassionate, organised, and proactive Care Partner to join our growing team. This role is focused on supporting clients to remain living independently at home through effective care coordination, funding management, relationship management, and person-centred support planning.
Role Overview
As a Care Partner, you will manage an allocated client caseload and work closely with clients, families, support workers, nurses, allied health professionals, and the wider team to coordinate and monitor high-quality home care services.
You will play an important role in ensuring services are responsive, compliant, consumer-centred, and aligned with each client’s goals and changing needs.
Duties include (but are not limited to):
- Managing an allocated caseload of Support at Home and NDIS participants
- Developing and monitoring person-centred care plans
- Coordinating services in collaboration with rostering and support teams
- Monitoring client wellbeing and responding to changing needs
- Supporting clients and representatives to understand services, budgets, and available supports
- Managing client relationships, feedback, and complaints professionally and compassionately
- Liaising with support workers, contractors, nurses, allied health professionals, and external stakeholders
- Ensuring accurate documentation and case notes are maintained
- Reviewing services and goals regularly to ensure positive outcomes
- Supporting continuous improvement and quality compliance processes
- Providing guidance and support to care staff regarding client care needs and service delivery
Requirements
- Strong understanding of person-centred care and community care services
- Excellent communication and relationship-building skills
- Strong organisational and time management skills
- Ability to manage multiple tasks and competing priorities effectively
- Confidence working independently and collaboratively within a team
- Sound problem-solving and decision-making abilities
- Good computer literacy and documentation skills
- Victorian Driver Licence and reliable vehicle with insurance
- Current National Police Check (less than 3 months old) or NDIS Worker Screening Check
- Current First Aid and CPR Certificate
- Nursing experience is preferred but not necessary
- Ability to speak Khmer or Serbian is an advantage
