About the Role
Join a supportive multidisciplinary team and help patients and families navigate complex care. You’ll manage psychosocial needs, build strong links across sites and community, and create practical pathways that support safe, person centred outcomes across CALHN.
Duties
- Assess psychosocial needs of patients, families and carers
- Deliver counselling, consult and liaison across care teams
- Join ward rounds, case meets and shared care planning
- Use service and community resources to support best outcomes
- Collect data, report outcomes and support quality and research
- Contribute across sites to grow and shape social work services
About You
You’re a capable, calm and collaborative Social Worker ready to step into a busy health setting and make a real difference for patients, families and teams.
- Hold a Social Work degree with AASW full membership eligibility
- Apply health social work skills across complex, fast paced care
- Build trust and work well with multidisciplinary teams
- Communicate clearly with patients, carers and community partners
- Manage priorities, work autonomously and stay accountable
