Your new role:
As a Senior Social Worker within the Transitional Rehabilitation Program, you will play a pivotal role in supporting people with spinal cord injuries as they transition from inpatient rehabilitation to community living. Working within a highly skilled interdisciplinary team, you'll provide advanced clinical care while contributing to service innovation, education and quality improvement initiatives.
You will,
Deliver high-quality Social Work assessment, intervention, discharge planning and support services to patients and their families throughout the rehabilitation journey.
Provide clinical leadership, expert advice and guidance to team members while supporting less experienced clinicians, students and Allied Health Assistants.
Collaborate with healthcare professionals, community organisations and external stakeholders to achieve positive patient outcomes and seamless transitions of care.
Contribute to service development, quality improvement and research activities that enhance patient care and rehabilitation outcomes.
About you:
You are a compassionate and experienced Social Worker who thrives in complex clinical environments and is passionate about supporting people to achieve meaningful goals and independence.
You will have,
Demonstrated experience delivering advanced Social Work services within rehabilitation, disability, health or complex care settings.
Strong clinical reasoning, assessment and case management skills, with the ability to manage competing priorities and complex patient needs.
Excellent communication and relationship-building skills, with experience working collaboratively across multidisciplinary teams and community networks.
Proven ability to mentor, supervise and support developing practitioners while contributing to service improvement and evidence-based practice.
