About the Role
We have an opportunity available for a relationship-driven Case Manager to join our Moorabbin-based team, supporting older Australians in the Bayside area to live independently, safely and confidently in their own homes.
This is a varied and rewarding role that combines office-based care coordination with face-to-face client interaction. You’ll partner closely with clients and families to understand their goals, determine their care team, manage funding and budgets, and adapt services as client needs change over time.
Working across privately funded clients and those who receive government funded Support at Home Packages, you’ll oversee the full case management journey, from intake and onboarding through to ongoing care reviews, funding management and service coordination. You’ll also collaborate with allied health providers and other external stakeholders to ensure every client receives high-quality, personalised support.
About You
You enjoy building genuine relationships, thrive in a fast-paced environment and bring a calm, organised and solutions-focused approach to your work. Passionate about supporting older people to live well at home, you're highly organised, adaptable and capable of managing competing priorities, funding and budgeting requirements, and navigating complex situations.
You will bring:
- Previous Case Management or community care experience
- A working knowledge of Support at Home Package funding and budget management
- Clinical knowledge and confidence managing changing client needs and complex situations
- Excellent time management and organisational skills
- Exceptional communication and relationship-building capability
- Strong attention to detail and compliance-focused documentation skills
- A collaborative and team-oriented approach
- Willingness to travel for client visits across the community
- Qualifications in Aged Care, Community Services, Disability, Nursing or Allied Health will be highly regarded
