Leadership Opportunity Across Victoria's Healthscope Rehabilitation Hospitals
An exciting opportunity exists for a motivated and enthusiastic Registered Nurse or Allied Health Professional to lead our Rehabilitation Assessment Service across Healthscope's Victorian Rehabilitation Hospitals.
This role is ideal for an experienced rehabilitation clinician who is passionate about patient-centred care, building strong referral relationships, and leading a high-performing assessment team. Working across metropolitan Melbourne, you will play a vital role in ensuring patients are appropriately assessed and admitted to rehabilitation services that support their recovery and independence.
About the Role
As the Rehab Assessment Service Lead, you will be responsible for overseeing a team of assessment clinicians who review referrals and conduct patient assessments within acute hospitals and community settings. You will drive service growth, build strong stakeholder relationships, and ensure a seamless patient journey into rehabilitation services.
Key Responsibilities

Lead and support a team of rehabilitation assessment clinicians across multiple catchment areas throughout metropolitan Melbourne.

Coordinate the timely review, assessment and admission of prospective rehabilitation patients into our hospitals.

Conduct patient assessments and provide clinical recommendations regarding rehabilitation suitability.

Promote rehabilitation services and identify opportunities for service growth.

Develop and maintain effective relationships with referring hospitals, medical specialists, nursing staff, allied health professionals and other key stakeholders.

Monitor referral activity and admission pathways to optimise patient flow and service outcomes.

Provide coaching, mentoring and professional development opportunities for team members.

Ensure assessments are conducted in accordance with clinical standards, organisational policies and rehabilitation best practice

Essential Criteria:

Tertiary qualifications in Nursing or an Allied Health discipline.

Current registration with AHPRA.

Extensive clinical experience within a rehabilitation hospital or rehabilitation service setting.

Previous leadership or management experience, with the ability to motivate and develop a high performing team.

Strong understanding of rehabilitation models of care, patient selection criteria and rehabilitation pathways.

Exceptional relationship building and stakeholder engagement skills.

Excellent written, verbal and interpersonal communication skills.

Strong organisational skills with the ability to manage competing priorities.

Current Victorian Driver's Licence and willingness to travel across metropolitan Melbourne.
