About the Role
We are seeking a compassionate and experienced Support Worker to provide high-intensity NDIS supports to a 65-year-old male participant living with Parkinson's disease in Mount Albert.
The participant lives with his wife and requires assistance with daily activities, mobility, and community access. This is an opportunity to build a positive and supportive working relationship with both the participant and his family.
Support Worker – High Intensity Supports (NDIS)
Location: Mount Albert
Participant: Male, 65 years old
Hours: Approximately 6 hours per day
Commencement: Within the next few weeks
Key Responsibilities
- Provide high-intensity NDIS supports in accordance with the participant's care plan.
- Assist with transfers and mobility support.
- Support the participant with activities of daily living.
Assist with community access and social participation. Not necessarily the worker needs to have a car
- Monitor changes in mobility and wellbeing and report concerns as required.
- Work collaboratively with the participant, family members, and other support professionals.
- Maintain accurate documentation and progress notes.
Participant Information
- Diagnosis: Parkinson's disease.
- Uses a wheelchair for mobility.
- Has a hospital bed in the home.
- Mobility is declining, and transfer support is required.
- A hoist may be required in the future as care needs increase.
Essential Requirements
- Experience supporting participants with complex physical support needs.
- Understanding of Parkinson's disease and mobility-related challenges.
- Experience with manual handling and transfer techniques.
- Current NDIS Worker Screening Check.
- Current First Aid and CPR Certificate.
- Valid driver's licence and reliable vehicle (preferred).
- Excellent communication and interpersonal skills.
Preferred
- Experience providing high intensity supports under the NDIS.
- Experience using mobility equipment and hoists.
