Vitium Cura is seeking an experienced, compassionate and participant-focused Support Coordinator to support NDIS participants in Bundaberg and surrounding areas.
About the Role
As a Support Coordinator, you will work alongside participants to help them understand and implement their NDIS plans, connect with appropriate services, strengthen their support networks and build their capacity to exercise choice and control.
You will collaborate with participants, families, service providers, allied health professionals and community organisations to support positive and sustainable outcomes.
Key Responsibilities
- Support participants to understand and implement their NDIS plans.
- Identify and connect participants with suitable NDIS, mainstream and community services.
- Coordinate providers and other stakeholders involved in participant supports.
- Support participants to build independence and capacity to manage their supports.
- Monitor plan implementation, service agreements and funding utilisation.
- Assist participants to prepare for plan reassessments/reviews and changes in circumstances.
- Identify service gaps, barriers and risks and assist with appropriate solutions.
- Maintain accurate case notes, reports and participant documentation.
- Build strong professional relationships with participants, families, providers and stakeholders.
- Maintain professional, person-centred and confidential practice.
What We’re Looking For
- Previous NDIS Support Coordination experience highly regarded.
- Strong understanding of the NDIS, participant choice and control and person-centred practice.
- Experience coordinating supports for participants with complex or multiple support needs desirable.
- Excellent communication, documentation and stakeholder-management skills.
- Ability to manage a caseload and work independently.
- Relevant qualification in Social Work, Community Services, Disability, Mental Health or a related discipline desirable.
- Appropriate NDIS worker screening and other applicable compliance requirements.
- Knowledge of services and community resources within Bundaberg and surrounding areas will be highly regarded.
