About the Role
Place of Hope Care is seeking a mature, experienced Male Support Worker to provide one-on-one community support to a participant in Carlton North.
The participant is a 40-year-old male living with a psychosocial disability and a history of substance use. He is intelligent, engaging, and responds well to workers who build trust through consistency, respect, and genuine connection.
This rewarding role involves supporting the participant to access the community, develop healthy routines, increase independence, and achieve their NDIS goals while maintaining clear professional boundaries.
Key Responsibilities
- Providing one-on-one community access and support with appointments and social activities.
- Encouraging healthy routines, positive lifestyle choices, and greater independence.
- Prompting and supporting medication in line with the participant's care plan.
- Building a trusting relationship while maintaining clear professional boundaries.
- Maintaining accurate documentation and communicating effectively with the Support Coordinator and Place of Hope Care management.
What We’re Looking For
We're seeking a mature, reliable, and empathetic support worker who is passionate about making a genuine difference in people's lives. The ideal candidate will have:
- Experience in disability support, mental health, or community services (essential).
- Certificate III or IV in Individual Support, Disability, Mental Health, or similar (preferred).
- Current NDIS Worker Screening Check.
- Current First Aid & CPR Certificate.
- Current Australian Driver's Licence and access to a reliable, insured vehicle.
- Strong communication skills and the ability to maintain professional boundaries.
