Leaving hospital is a milestone. Getting home safely and staying there is the real work — and that's where you come in.
As an Occupational Therapist with our Veterans and Aged Community Care team, you'll support clients through one of the most important transitions they'll make. Assessing what they need, building plans around how they actually want to live, and putting the right equipment, support and education in place so independence isn't just possible — it's sustainable. It's hands-on, it's meaningful, and the impact lands directly in someone's home.
This role is available part-time (0.6 FTE) through to full-time, and we're genuinely flexible on how that looks — because good people shouldn't have to choose between great work and a life outside of it.
What you'll be doing
- Conducting thorough client assessments and developing care plans built around individual goals and circumstances
- Providing functional support and rehabilitation that helps clients live independently and safely at home
- Prescribing and managing DVA equipment — including installation and client education — so people know how to use what they've been given
- Collaborating with hospitals, GPs, allied health professionals and funding bodies to make sure nothing important gets missed
- Keeping documentation accurate and meeting compliance standards without letting admin take over your day
- Contributing to quality improvement initiatives that make the whole team better
You're the right person if...
- You hold a Bachelor of Applied Science (Occupational Therapy) and current AHPRA registration with no conditions
- You bring solid experience in assessments, rehabilitation and equipment prescription
- You communicate with clarity and warmth — with clients, families and the wider care team
- You hold a current Driver's Licence and have a reliable vehicle
- You hold a Working with Children Check and NDIS Worker Screening Clearance, or you're ready to get one
