About the role
The Clinical Care Partner provides clinical leadership and care management support under the Support at Home Program, ensuring older people receive safe, effective and person-centred care that enables them to live independently at home for as long as possible. Working within a multidisciplinary team, you will provide clinical oversight across a caseload of participants, supporting complex care needs, managing clinical risks, and coordinating services that promote health, wellbeing and quality of life. You will work closely with Care Partners, allied health professionals, GPs, specialists, carers and families to ensure care plans are clinically appropriate and responsive to changing needs.
As a Care Partner, you will:
- Conduct comprehensive assessments to identify health risks, clinical needs and appropriate interventions.
- Collaborate with participants to develop and review individualised care plans that reflect their goals, service preferences, cultural needs and clinical requirements.
- Provide clinical oversight and guidance for participants with complex health needs, including chronic disease management, wound care, medication management and continence support.
- Liaise with GPs, specialists, hospitals and allied health providers to ensure coordinated and integrated care.
- Monitor participant wellbeing, clinical outcomes and changing needs, proactively identifying and managing risks.
- Support participants and their carers to make informed decisions about their care through education, advocacy and supported decision-making.
- Facilitate or contribute to case conferences and multidisciplinary reviews to ensure best-practice care planning.
- Collaborate with participants to plan and manage individualised budgets, ensuring services align with assessed needs and available funding.
- Monitor expenditure to minimise underspends and overspends while supporting optimal care outcomes.
- Maintain accurate clinical documentation and ensure all records meet organisational, legislative and regulatory requirements.
- Ensure service delivery aligns with the Support at Home Program and Strengthened Aged Care Quality Standards.
- Identify, report and escalate clinical incidents, risks and concerns appropriately.
To be successful, you will have:
- University-level qualification in a relevant health-related discipline, for example, Bachelor of Physiotherapy or Nursing (current AHPRA Registration preferred)
- Relevant Care Management and aged care experience in a similar role
- Highly developed assessment skills and the ability to creatively problem solve complex care
- Relevant understanding and experience in the delivery of health services and community-based care and support services
- Strong understanding of the Support at Home Program, Quality Standards, and aged care rights.
- Strong communication, empathy and cultural competence.
- AlyaCare experience highly desirable
- Australian driver's licence and access to a reliable car for work
