MCWA is a in-home aged care provider with 40 years experience that focuses on supporting the multicultural ageing and elderly communities in Australia. We are currently seeking compassionate and dedicated Case Manager/Care Partner to join our team based in the Melbourne, Australia.

Exciting opportunity for a person who is passionate about aged care, and supporting the ageing and elderly. The successful candidate will have a strong participant service focus with demonstrated ability to maintain strong ethics and care.

Some of the responsibilities for this position include:
- Demonstrated ability to build strong partnership relationships with participant
- Provide comprehensive case management for each participant
- Build strong Care Plans that are appropriate for the individual needs of each participant whilst meeting the limitations of each participant’s budget
- Re-assess participant care plans, goals, OHS assessment, and emergency plans for participants
- Maintain strong participant notes
- Strong problem solving skills
- Nimble in their ability to support participants
- Recognise, anticipate, and focus on specific participant needs and advocate where required
- Work with the Finance team to accurately submit participant claims to Services Australia
- Approve monthly participant statements prior to mail out
- Establish, coordinate and manage third party services for participants
About You
- Experience in a similar role
- Registered Nurse looking for a change
- Proficiency in computer skills
- Excellent communication and interpersonal skills
- Ability to work independently and as a part of a multidisciplinary team
- Valid driver's license

Job Type: Full-time
Pay: $73,000.00 – $80,000.00 per year
