Due to our continued growth, we have a vacancy available for a Disability Services Officer.
Position Description
As a Disability Services Officer at Love 2 Care, you will play a vital role in providing high quality care to our valued clients in their homes. You will also be supporting and empowering members of our community in helping them meet their relevant needs.
Key Responsibilities:
- Assess participants' individual support needs and work collaboratively with participants, their families, guardians and relevant stakeholders to develop and implement person-centred support strategies aligned with their goals.
- Provide guidance and support to participants with a range of disabilities, including those with complex support needs, to promote independence, wellbeing, community participation and improved quality of life.
- Monitor participants' progress, evaluate support outcomes and recommend adjustments to support strategies to ensure services remain responsive to changing needs.
- Liaise and collaborate with allied health professionals, medical practitioners, families, carers and external service providers to ensure coordinated and effective service delivery.
- Assist participants to access appropriate community, health, education, employment and other support services that enhance social inclusion and independence.
- Provide participants and their families with information, guidance and advocacy regarding available supports, community resources, service options and participant rights.
- Identify potential risks, respond appropriately to incidents and contribute to the implementation of positive behaviour support strategies and risk management plans in accordance with organisational policies and NDIS Practice Standards.
- Maintain accurate, timely and confidential participant records, progress notes, incident reports and other documentation in accordance with NDIS Practice Standards, legislative requirements and organisational policies.
- Build and maintain positive working relationships with participants, families, community organisations and external stakeholders to facilitate high-quality, person-centred service delivery.
- Participate in case reviews, team meetings, quality improvement initiatives and ongoing professional development to maintain professional knowledge and ensure the delivery of high-quality disability services.
Skill and duties required:
· AQF associate degree, Advanced Diploma or Diploma (ANZSCO Skill Level 2)
- Minimum of 2 years of experience working with individuals with disabilities, providing support and resources, and facilitating access to services and programs.
- Strong understanding of disability issues, including accessibility, inclusion, and the social model of disability.
- Excellent communication and interpersonal skills, with the ability to work effectively with individuals with disabilities, their families, and community partners.
- Strong organisational and time management skills, with the ability to manage multiple priorities and deadlines.
- Ability to work independently and as part of a team, with a commitment to collaboration and cooperation.
- Familiarity with disability services and programs, including government-funded programs and services.
- Basic computer skills and experience with database management and record keeping.

- Experience within the Disability Services Industry

Salary and Benefits
Annual salary: $80,000 - $85,000 (depending on experience and qualifications).
Superannuation contributions in accordance with Australian law.
