We are a established community services provider supporting clients under the NDIS (National Disability Insurance Scheme) and Home Care Packages (HCP). We work with complex, high-needs clients and expect excellence from our team. We are now looking for a high-performing Senior Community Worker to manage a demanding caseload and drive service outcomes.
The Role
We are seeking an exceptional and highly experienced Senior Community Worker to manage a complex portfolio of NDIS and HCP clients. This is not an entry-level or standard coordination role. You will handle high-intensity case management, complex crisis intervention, multi-stakeholder coordination, and strategic service planning. You will also be expected to mentor junior staff and contribute to organisational quality improvement.
This role is suited to a current Senior Support Coordination Manager or an experienced community worker ready for genuine senior responsibility.
Key Responsibilities
Client Assessment & Complex Case Management
- Conduct comprehensive, biopsychosocial assessments for clients with complex needs, including mental health, behavioural challenges, housing instability, and multi-morbidity.
- Develop, implement, and review highly individualised support plans that address challenging behaviours, health risks, and social disadvantage.
- Manage multiple high-risk cases simultaneously, prioritising based on urgency and client vulnerability.
Coordination & Stakeholder Management
- Handle full coordination of supports across NDIS and HCP funding streams, including budget management, service booking, and compliance with price guides and legislation.
- Lead multidisciplinary collaboration with allied health professionals, GPs, housing services, legal advocates, and government agencies.
- Negotiate with difficult or non-responsive providers to ensure client needs are met without service gaps.
Crisis & Risk Management
- Lead crisis response for clients experiencing safeguarding issues, health deterioration, or behavioural escalation.
- Conduct thorough risk assessments and implement safety plans, including mandatory reporting and liaison with emergency services when required.
- Manage client incidents, complaints, and service failures with professional accountability.
Leadership & Quality Improvement
- Mentor, guide, and provide informal supervision to junior community workers and support coordinators.
- Identify systemic service gaps and propose practical improvements to management.
- Contribute to policy development, audit preparation, and continuous quality improvement initiatives.
Documentation & Compliance
- Maintain immaculate case notes, risk assessments, and communication logs in line with NDIS/HCP standards, privacy laws, and organisational policy.
- Prepare detailed reports for internal reviews, external auditors, or regulatory bodies as required.
What We Are Looking For
Essential (Non-negotiable):
- Minimum 1 year of full-time relevant experience in a senior or high-complexity community worker, support coordination, or case management role. Part-time counted pro-rata.
- Proven experience working with NDIS and HCP clients and handling complex coordination independently.
- A relevant qualification (minimum Diploma; Bachelor degree strongly preferred in Community Services, Social Work, Psychology, or Welfare).
- Demonstrated ability to manage crisis situations, high-risk clients, and challenging stakeholder relationships without constant supervision.
- Strong understanding of person-centred practice, cultural safety, trauma-informed care, and legal obligations (including mandatory reporting).
- Exceptional written and verbal communication skills, including the ability to write clear, detailed case notes and reports.
- Current [Police Check / Working with Children Check / NDIS Worker Screening Check / Driver's Licence – select as applicable].
Highly Desirable:
- Minimum 3+ years full-time equivalent post-qualification experience in community services.
- Experience supervising or mentoring other staff.
- Knowledge of NDIS practice standards, HCP guidelines, and relevant state-based community services legislation.
- Training in crisis intervention (e.g., Safe Crisis Management), mental health first aid, or behaviour support.
