Grace Quality Care is seeking a reliable, friendly and proactive NDIS Support Worker to provide regular community access and transport support to a male participant in the Dandenong area.
Our client is an independent man in his early 50s who is able to manage his own personal care and drive. He has strong family and community connections and enjoys getting out into the community. He would like to become more active, increase his community participation and have consistent support to assist him with appointments, activities and day-to-day outings.
This role would suit a support worker who is personable, dependable and comfortable spending time out in the community. Both male and female applicants are welcome, and there is no particular age preference.
Available Shifts
We are seeking someone with consistent availability on:
- Wednesday – approximately 4–6 hours
- Thursday – approximately 4–6 hours
- Friday – approximately 4–6 hours
- Expect approximately 5 hours of support per day on average
- Ongoing regular shifts for the right person
- Kilometres travelled while providing approved participant support will be reimbursed
Consistency is important, so preference will be given to applicants who can commit to all three days on an ongoing basis.
What You Will Be Doing
The role will primarily involve transport assistance, community access and social support, including:
- Supporting the participant to attend appointments and commitments
- Providing transport and assistance with travel within the community
- Supporting community access and participation
- Assisting with shopping, errands and other day-to-day activities
- Encouraging the participant to get out of the house and participate in activities of his choice
- Supporting the participant to build greater independence and confidence within the community
- Providing companionship and positive social engagement
- Assisting the participant to work towards his NDIS goals
- Maintaining appropriate shift notes and documentation following each shift
- Communicating any relevant concerns or changes to the Grace Quality Care team
This is not primarily a personal care role. The participant is relatively independent and is looking for a worker who can support him to get out and about, attend activities and appointments, and make better use of his community supports.

Essential Requirements
Applicants should have, or be willing to obtain where applicable:
- Current NDIS Worker Screening Check
- Working with Children Check, where required
- Current First Aid and CPR
- Valid Australian driver's licence
- Reliable, registered and insured vehicle suitable for participant transport
- Comprehensive vehicle insurance
- NDIS Worker Orientation Module
- Relevant experience in disability, community services, mental health or a similar support environment
- Ability to complete clear and professional progress/shift notes
