At Better Health Network, we believe in delivering accessible, inclusive and person-centred care that empowers communities to thrive. As part of the Better Homes Better Health program, you will play a key role supporting public housing residents as they prepare to relocate and settle into new homes as part of Victoria’s Big Build redevelopment initiative.
This is a unique opportunity to combine your Occupational Therapy expertise with meaningful community impact — supporting vulnerable residents to maintain independence, safety and wellbeing during significant life transitions.
About the role
Working within the Better Homes Better Health team, you will provide comprehensive Occupational Therapy assessments and interventions for residents living in public housing high-rise communities.
You will support individuals with complex health, social and functional needs through:
- Home and functional assessments
- Activities of daily living (ADL) support
- Home modifications and assistive equipment recommendations
- Relocation and settlement planning
- Education and self-management support
- Collaborative care planning with residents, carers and multidisciplinary teams
You will work closely with Homes Victoria, internal support teams and community stakeholders to ensure residents are supported throughout the relocation process and can continue living safely and independently within the community.
What you’ll bring

To succeed in this role, you will have:
- Tertiary qualification in Occupational Therapy
- Current AHPRA registration
- Experience working with older people, socially disadvantaged communities and/or people at risk of homelessness
- Experience conducting home assessments, prescribing equipment and recommending home modifications
- Knowledge of My Aged Care, CHSP, HACC PYP, Home Care Packages and/or NDIS
- Strong assessment, communication, problem-solving and relationship-building skills
- Eligibility or registration with SWEP and Gear 2 Go
