About the Role
Join the CO-ACT team and support older people transitioning into community or residential aged care supports. You will help deliver safe, person-centred care, build strong team connections and maintain quality support across the patient journey.
What you’ll do:
- Support nurses to deliver patient care
- Assist with daily care needs and comfort
- Observe, report and record patient changes
- Maintain safe, clean care spaces
- Build strong team relationships
- Communicate with patients, families and partners
- Follow care plans under RN or EN guidance
- Contribute to smooth patient transitions
About You
You’re ready to step into a supportive care role and make a real difference to older people as part of a collaborative team.
- Enrol in nursing degree or EN course, or hold Cert III or IV
- Communicate clearly with patients, families and teams
- Work well in a multidisciplinary team
- Deliver person centred care with empathy and respect
- Understand AIN role, standards, codes and safe practice
