Established in 2018, BJ Care Services Pty Ltd is a registered NDIS provider specialising in person-centred disability support for individuals with complex behaviours of concern. Led by directors with over 30 years of combined experience in the disability sector, we deliver structured, high-quality community and residential support services across Melbourne and Victoria.
Our services include community participation programs, social and community access support, NDIS school holiday programs, life skills development, Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Short-Term Accommodation (STA), and Respite Care. About the Role
We are seeking an experienced Community Worker to facilitate community development initiatives and coordinate collective solutions that address the recreational, health, housing, employment, and welfare needs of our NDIS participants. This role is primarily community-facing — the Community Worker operates across community settings, service networks, and external agencies to connect participants with the resources, programs, and opportunities they need to thrive.
A key focus of this role is the coordination and delivery of BJ Care Services' social and community participation programs, including our NDIS school holiday programs. These programs support participants — including children and young people with disability — to engage meaningfully with their communities, develop social skills, and access recreational and educational opportunities during school holiday periods.
Key Duties and Responsibilities
Job Responsibilities includes but not limited to:-
- Assess community need and identify available resources relating to health, welfare, housing, employment, training, and social participation for NDIS participants across Melbourne and Victoria
- Liaise with community groups, welfare agencies, government bodies (including the NDIA), schools, allied health professionals, families, and private organisations to promote awareness of community resources and coordinate service delivery
- Plan, coordinate, and deliver social and community participation programs, including NDIS-funded school holiday programs for children and young people with disability
- Assess individual participant needs and develop community-based educational, training, and support programs aligned with each participant's NDIS goals
- Interview participants and assess the nature and extent of their difficulties, including functional, social, emotional, and community access barriers
- Facilitate community development initiatives that promote social inclusion, independence, and active participation for people with disability across all age groups
- Organise and oversee recreational, educational, and social activities within community settings, ensuring programs are engaging, safe, and aligned with NDIS Practice Standards
- Monitor and report on participant progress against community participation and program goals, maintaining accurate records, case notes, and reports
- Refer participants to appropriate community agencies, allied health services, housing supports, and government programs that can provide additional assistance
- Assist participants to address social, emotional, and personal challenges through community-based supports and a strengths-based, person-centred approach
- Prepare submissions, funding requests, program reports, and correspondence for government bodies, NDIS planners, and external agencies
- Promote awareness of available community services, social participation opportunities, and school holiday programs among participants, their families, and the broader community
- Ensure all programs and services comply with the NDIS Practice Standards, the NDIS Code of Conduct, and BJ Care Services' organisational policies
Qualifications and Experience Required
Minimum requirements for the candidates
- A Diploma or Certificate IV in Community Work or Disability is essential.
- A minimum of one year of professional experience in a Community Work role or a comparable position is required.
Essential:
- Demonstrated experience working with individuals with disability, complex behaviours of concern, or in community welfare settings
- Strong understanding of the NDIS framework, NDIS Practice Standards, and person-centred care principles
- Experience assessing client needs and developing and implementing support or community participation programs
- Proven ability to liaise with community organisations, government agencies, allied health professionals, and families
- Excellent communication, documentation, and interpersonal skills
- NDIS Worker Screening Check (or willingness to obtain)
- Current First Aid and CPR certification (or willingness to obtain)
- Current Australian driver's licence
