About the Role:
Western Health is seeking an experienced, enthusiastic and motivate Grade 2 Clinical Pharmacist to join our dynamic Pharmacy Team supporting Emergency Medicine and Intensive Care services.
This is a permanent full-time (40 hours per week) role providing high-level clinical pharmacy services within the Emergency Department and broader critical care environment. You will work closely with the Emergency Medicine and Intensive Care Unit pharmacy teams to optimise medication management and support safe, effective, and evidence-based use of medicines.
This is an exciting opportunity for a pharmacist who is passionate about delivering high quality, patient-centred clinical pharmacy services within a fast paced, complex clinical environment. The successful applicant will play an integral role in improving patient outcomes through Quality Use of Medicines (QUM) and medication safety initiatives, research projects and multidisciplinary collaboration to deliver exceptional care to critically unwell patients.
Working alongside experienced clinicians across the Emergency Medicine and Intensive Care services division, the successful candidate will have the opportunity to develop advanced clinical skills, contribute to service improvement initiatives and make a meaningful impact on patient outcomes.
Key Responsibilities:
In this role, you will:

Support the Senior Pharmacists – Emergency Medicine and Senior Pharmacists – Intensive Care Unit in delivering critical care pharmacy services across Western Health

Provide expert clinical pharmacy services to patients in Division of Emergency Medicine and Intensive Care Services

Participate in medication safety, quality improvement and research activities.

Work collaboratively within multidisciplinary teams to deliver safe, high-quality, patient-centred care

Complete medication reconciliation, medication reviews, and clinical interventions for high priority patients

Contribute to education and training of pharmacy staff, students, and clinical teams
About You:
You are a collaborative and patient focused clinical pharmacist who enjoys working in fast paced environments and contributing to high quality clinical care.
You will bring:

Minimum 3 years’ broad hospital pharmacy experience across emergency medicine or a broad range of clinical specialities such as general medicine, cardiology, surgical services, or similar

Flexibility to work within an extended hour shift model (8-hour shifts between 7:00 am and 9:00 pm), including weekends and rotate through Western Health sites

Strong clinical judgement with the ability to work independently and collaboratively within a multidisciplinary team

A demonstrated passion for patient safety, evidence-based practice, and quality improvement

Well-developed verbal and written communication skills, with the ability to influence and engage a range of stakeholders
