Right at Home Adelaide Central is seeking an experienced, compassionate, and motivated Enrolled Nurse / Clinical Care Partner to join our growing clinical team.
This is an exciting opportunity for an experienced Enrolled Nurse who is passionate about supporting older Australians to remain independent, safe, and well within their own homes.
As a Clinical Care Partner, you will play an important role in conducting client assessments, developing and reviewing care plans, supporting quality and compliance activities, mentoring care staff, and coordinating the delivery of high-quality community-based care under the Support at Home Program, Home Care Packages, and private services.
Working closely with our Registered Nurses and multidisciplinary team, you will contribute to the delivery of safe, person-centred care that promotes independence, wellbeing, and quality of life.
About the Role
You will work collaboratively with clients, families, care workers, allied health professionals, General Practitioners, and external stakeholders to ensure services are clinically appropriate, person-centred, and aligned with the Aged Care Quality Standards.
This role combines clinical nursing, care coordination, client relationship management, and quality support within a supportive multidisciplinary environment.
Key ResponsibilitiesClinical Assessment & Care Planning
- Conduct comprehensive client assessments within the Enrolled Nurse scope of practice.
- Assist in developing, implementing, and reviewing individualised care plans.
- Identify clinical risks and escalate concerns to the Registered Nurse or Clinical Manager where appropriate.
- Support reablement, restorative care, and wellness-focused approaches.
- Monitor client wellbeing and recommend service adjustments.
- Complete accurate and timely clinical documentation.
Clinical Support
- Provide clinical guidance and support to care workers.
- Assist with supervision and competency observations of care staff.
- Support medication management processes in accordance with organisational policies and legislative requirements.
- Participate in wound care, continence management, chronic disease support, and other nursing interventions within your scope of practice.
- Promote evidence-based practice and continuous quality improvement.
Client & Family Support
- Develop positive relationships with clients, families, and representatives.
- Support clients to achieve their goals and maintain independence.
- Respond professionally to enquiries, concerns, incidents, and complaints.
- Participate in multidisciplinary case conferences and client reviews.
Quality, Compliance & Clinical Governance
Assist the clinical team in ensuring compliance with:
- Aged Care Quality Standards
- Support at Home Program requirements
- Clinical Care Standards
- Infection Prevention and Control requirements
- Organisational policies and procedures
Responsibilities include:
- Maintaining accurate clinical records.
- Participating in incident reporting and investigations.
- Supporting risk management activities.
- Assisting with internal audits and continuous improvement initiatives.
- Supporting accreditation and compliance activities.
Care Coordination
- Collaborate with rostering staff to ensure services align with assessed client needs.
- Assist with client reviews and package utilisation.
- Support onboarding of new clients and commencement of services.
- Liaise with clients, families, allied health professionals, and external providers to ensure continuity of care.
After-Hours Support
- Participate in a rotating clinical on-call roster where applicable.

About You
You are an experienced Enrolled Nurse who is passionate about community nursing and supporting older Australians to live independently with dignity, choice, and respect.
Essential Requirements
- Current AHPRA Registration as an Enrolled Nurse.
- Minimum 2–3 years' experience as an Enrolled Nurse, preferably in community nursing, aged care, or primary health care.
- Experience in one or more of the following:
- Support at Home
- Home Care Packages
- Community Nursing
- Residential Aged Care
- Disability Services
- Rehabilitation
- Chronic Disease Management
- Strong clinical assessment and observation skills.
- Experience contributing to care planning and clinical documentation.
- Excellent communication and interpersonal skills.
- Strong organisational and time-management abilities.
- Ability to work independently while recognising and working within the Enrolled Nurse scope of practice.
- Current National Police Check.
- Current First Aid and CPR Certificate.
- Current unrestricted Driver's Licence.
- Reliable motor vehicle.
- Comprehensive motor vehicle insurance.

Desirable
- Experience working within the Support at Home Program and Home Care Packages.
- Knowledge of the Aged Care Quality Standards.
- Experience using VisualCare or similar client management software.
- Experience supporting quality improvement activities.
- Skills in wound care, continence management, dementia care, palliative care, or chronic disease management.
- Certificate IV in Training and Assessment or experience mentoring care staff (desirable).

Salary: $75,000 – $85,000 per year (dependent on experience)
