About the role
As an experienced Nephrology Nurse Practitioner at RMH, you will provide clinical leadership in the delivery of effective, timely and person-centred care to people living with chronic kidney disease (CKD), supporting the management of progressive kidney disease and the transition to kidney failure where required.
You will empower patients and their families through education, clinical support and shared decision-making, helping them navigate their care pathway and make informed choices aligned with their values and preferences.
Working autonomously and collaboratively, you will partner with nephrologists and a broad multidisciplinary team, including acute care, palliative care, primary health and community-based services, to ensure coordinated and comprehensive care. You will also contribute to research, education and quality improvement initiatives that strengthen evidence-based practice and enhance the delivery of person-centred kidney care.
Key Accountabilities
Provide advanced and autonomous clinical assessment, management and care for patients with CKD and kidney failure.
Lead person-centred care planning, education and decision support across the kidney care pathway.
Identify and manage complex clinical needs, complications and treatment-related issues, including medication management and supportive care.
Lead multidisciplinary case conferences and collaborate with nephrology, palliative care, primary and community services to coordinate care.
Provide clinical leadership, education and support to nursing staff and health professionals across RMH and community services.
Lead the development of evidence-based clinical practice, policies and pathways, contributing to research, quality improvement and service innovation.
About You
You are an experienced Nephrology Nurse Practitioner with strong clinical expertise in chronic kidney disease and a commitment to delivering person-centred, values-based care. You bring advanced clinical practice, strong multidisciplinary collaboration and the ability to lead, educate and influence improvements in kidney care.
You will have:
Current AHPRA registration as a Registered Nurse and an approved Master of Nurse Practitioner qualification or be working towards one.
Minimum five years’ experience in nephrology and/or palliative care.
Demonstrated advanced clinical assessment, critical thinking and decision-making skills in complex patient care.
Proven ability to provide high-quality, person-centred care and work collaboratively across multidisciplinary teams.
Strong communication, leadership and stakeholder engagement skills, with experience supporting education, research and quality improvement.
A commitment to The Melbourne Way, with the ability to lead innovation, adapt to change and drive evidence-based practice.
