About the Role:
Coolibah Care is seeking an experienced and passionate In-Home CHSP Coordinator to join our Home & Community Care team.
This is a diverse role combining care coordination, team leadership and direct client support. You will play an important role in ensuring clients receiving services through the Commonwealth Home Support Programme (CHSP) receive high-quality, person-centred care that supports their independence, wellbeing and individual goals.
Working closely with our Care and Support Workers, Respite Coordinator, Clinical Care Partners, Schedulers and wider multidisciplinary team, you will coordinate client services from onboarding through to ongoing care planning and review.
You will also remain connected to frontline service delivery by providing direct care and support when required, assisting with new client commencements and providing short-term workforce coverage to ensure continuity of care.
What you need to be successful in this position:
- Minimum qualification of Certificate IV in Aged Care or Individual Support (Ageing). with experience in CHSP, Assessments & Team Leadership
- Passion for providing compassionate care and promoting independence.
- Excellent interpersonal and communication skills.
- Reliability, patience, and adaptability to meet diverse needs.
- Full Driver's Licence and reliable registered vehicle.
- Current Police Clearance (less than 12 months) and First Aid Certificate (or willingness to obtain).
- It is recommended that you will have 2 COVID Vaccinations and an up-to-date Flu vaccination

- Experience in aged care, community care, home care or a similar service environment.
- Experience coordinating care and developing or reviewing individualised care plans.

- A strong understanding of person-centred care, independence and reablement.
- Excellent organisational and documentation skills.
