NDIS Community Access Support Worker – Campbelltown
Harmony Home Disability Services is seeking a reliable, friendly and engaging Support Worker to provide community access and social support to an NDIS participant based in the Campbelltown area.
This role is primarily focused on supporting the participant to access the community, enjoy activities aligned with his interests and build greater social connections and independence.
Support Activities May Include
- Visiting local community venues and pubs
- Watching football and attending Wests Tigers home games
- Going to the movies, particularly action films
- Community outings and walks
- Social and companionship-based support
- Supporting the participant to build social connections
- Encouraging increased community participation and independence
- Assisting with errands and appointments
- Supporting medication collection from a pharmacy in Sefton every Wednesday morning
- Providing occasional support within the participant’s home, where required
Availability Required
Support is mainly required:
- Wednesday to Saturday
- Every Wednesday from approximately 8:00 am for medication collection
- Occasional Monday or Tuesday afternoons
- Immediate commencement is preferred
Essential Requirements
Applicants must have:
- A current NDIS Worker Screening Check
- A current First Aid and CPR certificate
- A current Australian driver’s licence
- Access to a reliable, registered and insured vehicle
- Comprehensive vehicle insurance
- Good communication and interpersonal skills
- The ability to complete accurate shift notes and documentation
- A person-centred and respectful approach
- Reliability, punctuality and flexibility
Previous experience providing disability, community access or social support is preferred.

- Reliable and available for regular weekday and occasional weekend shifts
