About the opportunity
Reporting to the Integrated Community Manager, you'll provide senior clinical leadership across BallyCara's Support at Home Program, ensuring our clients receive safe, high-quality, person-centred care that supports them to remain living independently at home.
Based at our Kippa Ring office, you'll work closely with Care Partners, Allied Health professionals and our multidisciplinary team to provide clinical oversight, mentor staff and drive continuous improvement across the service.
This role offers the perfect balance of leadership, clinical governance and operational oversight, while still maintaining a connection to client care when required.
What you'll be doing
- Providing clinical leadership across BallyCara's Support at Home Program
- Supporting, coaching and mentoring Care Partners and clinical team members
- Overseeing complex assessments, care planning and service coordination
- Ensuring care is delivered in line with client goals, funding and legislative requirements
- Driving clinical governance, quality improvement and compliance
- Building strong relationships with clients, families and healthcare professionals
- Supporting service growth and delivering exceptional client outcomes
Skills and Experiences:
About you
You're a compassionate and confident Registered Nurse who enjoys leading others and improving the quality of care.
You'll also bring:
- Current AHPRA registration as a Registered Nurse
- Strong understanding of the Support at Home Program and aged care reforms
- Experience coordinating complex community-based clinical care
- Demonstrated leadership, coaching and mentoring experience
- Excellent communication and stakeholder engagement skills
- Strong clinical judgement and problem-solving abilities
- Exceptional organisational and documentation skills
- A passion for person-centred care and continuous improvement
