About the role:
The Care Partner plays a vital role in supporting our clients to remain living independently, safely and comfortably in their own homes through capacity building, strengthening connections and utilising the Support at Home or NDIS Funding to achieve their goals.
The successful applicant will:
- Develop, in collaboration with the client and their support network, a Care Plan, Budget and Service Agreement that meets the client’s needs and goals.
- Demonstrate a proactive approach to assessment, planning and delivery of support options that are person-centred, wellness and reablement focused and build on the strengths, capacity, goals and interests of clients.
- Conduct individual risk assessments in relation to clients needs, supports and home environments and ongoing management of identified risks.
- Continuously build a strong client base, develop trusted relationships with existing and prospective clients, and proactively support clients to fully understand and optimise utilisation of their package to meet their needs and goals.
- Coordinate, review and monitor the delivery of supports and services identified in the client plan, ensuring they remain relevant to the individual’s needs.
- Creatively address barriers and encourage actions that promote a level of independence, reduce risk and ensure people remain well for as long as possible.
- Maintain accurate and comprehensive records, following policies and procedures, including client records.
- Ensure adherence to all legislative and regulatory requirements.
About You
Essential
This role requires:
- Relevant tertiary qualifications ( Aged Care related disciplines including Diploma of Nursing and/or Cert III in Aged Care, Disability or Community Services) or demonstrated experience in Community Services.
- Demonstrated experience in case management or support coordination, including the assessment of clinical and care related risks.
- Understanding of, and commitment to, person-centred and strengths-based practices and wellness and reablement principles.
- Superior customer service and engagement skills, with the ability to continuously build a strong client base and develop solid relationships with existing and potential clients.
- Knowledge and understanding of the Aged Care Sector, the Support at Home Program and the NDIS.
- Demonstrated experience using Microsoft Office Suite and Client Management Systems.
- Demonstrated ability to work collaboratively and build strong and productive working relationships both internally and externally.
- Excellent organisational and time management skills, with a high attention to detail and ability to establish priorities.
- Excellent communication skills, both verbal and written.
