About the Role
WeCare Wellness is seeking an experienced and compassionate Care Partner to join our Support at Home team.
In this role, you will work closely with older people and their families to understand their individual needs, goals and preferences, coordinate appropriate services, and support them to live safely and independently at home.
As many of our clients come from Chinese-speaking backgrounds, fluency in Mandarin and/or Cantonese is essential for this position.
Key Responsibilities
- Build and maintain strong relationships with clients and their families.
- Conduct client consultations and understand individual care needs, goals and preferences.
- Develop, implement and regularly review individualised care plans.
- Coordinate Support at Home services, including personal care, domestic assistance, transport, social support, allied health and other approved services.
- Monitor client wellbeing, changing needs and service outcomes.
- Work closely with clients, families, support workers, allied health professionals and other service providers.
- Assist clients to understand their services, available funding and service arrangements.
- Maintain accurate client records, case notes and relevant documentation.
- Identify risks, changes in circumstances and potential safeguarding concerns, and escalate appropriately.
- Ensure services are delivered in accordance with organisational policies, relevant aged care requirements and quality standards.
- Support clients through changes associated with the Support at Home program.
- Deliver culturally appropriate and person-centred support to Chinese-speaking older people.
About You
To be successful in this role, you will ideally have:
- Fluent Mandarin and/or Cantonese – essential
- Strong English communication skills
- Previous experience in aged care, home care, case management, care coordination or a similar role
- Good understanding of person-centred care and the Australian aged care system
- Knowledge of the Support at Home program will be highly regarded
- Strong client relationship and communication skills
- Excellent organisation, time management and follow-up skills
- Ability to manage multiple clients and priorities independently
- Strong problem-solving skills and attention to detail
- Confidence communicating with older people and their families from culturally diverse backgrounds
- Relevant qualifications in aged care, community services, nursing, social work, allied health or a related discipline will be highly regarded
- Current Police Check and other employment screening as required for the role

Pay: $27.00 – $40.00 per hour
