About the Role
Prime Care Ability is seeking a reliable and experienced Registered Nurse (RN), Enrolled Nurse (EN), or suitably qualified medication-trained care professional to provide regular in-home support to a client living with dementia and requiring daily insulin administration.
This is an ongoing community-based role requiring a person who is dependable, clinically competent and comfortable working with a client who requires medication support and supervision at his home.
Key Responsibilities
- Attend the client's home for a 2-hour shift
- Provide daily insulin administration in accordance with the client's prescribed medication/diabetes management plan and within the worker's scope of practice
- Monitor the client for relevant signs and symptoms and report any concerns appropriately
- Support safe medication administration and maintain accurate medication records
- Provide appropriate support to a client living with dementia
- Monitor and report changes in the client's condition, behaviour or wellbeing
- Follow the client's care plan, medication orders and clinical instructions
- Maintain appropriate documentation after each visit
- Communicate with family members and the care team when required
- Report medication incidents, missed doses, adverse reactions or other clinical concerns promptly
- Maintain client privacy, dignity and confidentiality
- Follow all relevant organisational policies, procedures and infection-control requirements
Essential Requirements
Applicants must have one of the following:
- Registered Nurse (RN) with current AHPRA registration; or
- Enrolled Nurse (EN) with current AHPRA registration; or
- Appropriate and recognised medication administration qualification/competency, such as:
- HLTHPS007 – Administer and Monitor Medications, or
- CHCCS424B – Administer and Monitor Medications, or
- An appropriate recognised equivalent/current medication administration competency.
Applicants must be appropriately trained, competent and authorised to administer insulin within their scope of practice and applicable requirements.
Other Requirements
- Experience with insulin administration and diabetes management
- Experience supporting people with dementia is highly desirable
- Strong understanding of medication safety
- Good clinical observation and reporting skills
- Excellent communication and documentation skills
- Ability to work independently in a client's home
- Current First Aid and CPR
- NDIS Worker Screening Check and/or Police Check, as applicable
- Australian working rights
- Reliable transport
- Professional, punctual and dependable
Availability
This role requires consistent availability across 7 days of the week.
The shifts are approximately 2 hours per day, with the exact shift time to be discussed based on the client's care requirements.
