About the role
Provide high-quality, evidence-based nursing care within the Discharge Transit Centre (DTC) at the Royal Brisbane and Women's Hospital, supporting safe, efficient, and patient-centred transitions from hospital care. Working as part of the Patient Flow Service, this role plays a key part in facilitating timely discharge processes while ensuring patients continue to receive appropriate clinical care and support.
You will deliver contemporary nursing care through comprehensive assessment, planning, implementation, and evaluation, working both independently and collaboratively within an interprofessional team. The role requires the ability to respond effectively to changes in patient condition, provide patient and family education, and contribute to coordinated care delivery that supports optimal health outcomes.
As a Registered Nurse in the DTC, you will contribute to maintaining patient flow across the hospital by supporting safe discharge practices, reducing delays, and ensuring continuity of care. You will apply clinical judgement and problem-solving skills in a fast-paced environment, balancing clinical care with coordination and communication across multiple teams and service lines.
