Your Opportunity
An exciting opportunity to be part of a new and evolving service in Marapikurrinya South Hedland, supporting older people who require additional assistance to access aged care and community supports. As a Case Manager within the Care Finder program at Marapikurrinya South Hedland, you will provide intensive, personalised support to individuals experiencing vulnerability, including those without family networks, with limited literacy, or facing health, housing, or social challenges.
Working with a caseload of approximately 20 to 30 individuals, the position combines structured case management with practical, hands-on support such as assisting with essential needs, navigating systems, and building confidence to live more independently. This is an on-site position, focused on face-to-face engagement and outreach, enabling strong relationship building and deeper understanding of each person’s goals and circumstances. The role plays an important part in improving access to aged care services while strengthening connections between health, housing, and local community supports.
Your Key Responsibilities:

Deliver person centred case management to older individuals with complex needs, including assessments, care planning, and ongoing reviews.

Support clients to access aged care services through My Aged Care, including completing forms, attending assessments, and coordinating referrals.

Provide practical assistance such as accessing supports, housing applications, Centrelink, and medical supports to address immediate needs.

Build strong, trust based relationships to understand client goals, strengthen independence, and increase confidence navigating services.

Coordinate and advocate for access to internal and external services, including health, housing, and community supports.

Maintain accurate case notes, reports, and client documentation in line with program requirements and privacy standards.

Develop and maintain community networks, participate in meetings, and promote service integration across local systems.
Requirements for Success

Relevant degree with at least one year of experience, or an associate diploma with relevant experience, or equivalent industry experience.

Experience in case management with the ability to manage a caseload of clients with complex needs.

Strong understanding of community services, aged care, and housing or homelessness supports.

Ability to build relationships quickly and work effectively with vulnerable older people, including Aboriginal and Torres Strait Islander communities.

Well-developed communication, organisational, and time management skills with the ability to work autonomously.

Knowledge of trauma informed practice and the ability to maintain professional boundaries
-
Compassionate, adaptable, and proactive approach to supporting clients to build independence and remain connected to services.

First Aid & CPR certificate.
-
Full driver licence with no restrictions.
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
