STAR Community Services is expanding into Support at Home and we are seeking a Care Partner to join our dedicated team. In this role, you will act as the primary point of contact for participants, ensuring their care and services are aligned with their unique needs, goals, and preferences. This is an exciting opportunity to guide, support, and advocate for older Australians as they navigate their care journey.
Who is STAR Community Services?
STAR Community Services is an award-winning not-for-profit supporting elderly and people with disabilities since 1996. We provide affordable, reliable aged care and disability services—including Support at Home, NDIS, transport, social activities, and companionship—to help clients live independently and stay connected. With help from dedicated staff and local volunteers, we serve Redlands, Ipswich, Logan, Gold Coast, and Toowoomba. STAR is committed to building inclusive communities and combating social isolation through regular programs and events.
Purpose of the Role
The Care Partner plays a pivotal role in person-centred care planning, coordination, and advocacy. You will collaborate with participants, carers, clinically qualified care partners, and service providers to deliver safe, responsive, and outcome-focused care that promotes wellness, independence, and reablement.
Key responsibilities (not limited to)
Conduct comprehensive care planning, including client needs assessment, risk assessments, and development/review of individualised care plans and budgets
Support clients to understand and review their support plans, ensuring services align with their goals, preferences, and circumstances
Establish and manage service agreements with clients to ensure effective service delivery
Manage client budgets and service utilisation, including tracking via relevant systems and ordering required consumables
Build and maintain strong, respectful relationships with clients, carers, and stakeholders through clear and culturally appropriate communication
Monitor client wellbeing through regular reviews, care note analysis, and case discussions, responding promptly to changing needs
Support clients in informed decision-making, promoting independence, wellness, and reablement approaches
Collaborate with clinically qualified professionals, escalating complex or high-risk cases and supporting specialised care pathways
Maintain accurate, timely documentation and ensure compliance with aged care standards, policies, and regulatory requirements
Participate in quality improvement, audits, and continuous improvement initiatives

What will you bring to the role?
Certificate IV in Aged Care, Community Services, Disability, or Case Management (or equivalent).
Experience in care coordination within aged care, or community services.
Knowledge of aged care frameworks and quality standards.
Strong skills in participant engagement, risk assessment, and advocacy.
Digital literacy with experience using care management systems and office software.
