About the Role
We are seeking a motivated and experienced NDIS Support Coordinator to join our growing team. You will work closely with NDIS participants to help them understand and implement their plans, connect with appropriate services, and build the confidence and skills to exercise choice and control over their supports.
Key Responsibilities
- Support participants to understand and implement their NDIS plans and work towards their individual goals
- Connect participants with suitable disability, community and mainstream services
- Coordinate and monitor supports, service agreements and plan budgets
- Build strong relationships with participants, families, providers and other stakeholders
- Identify and respond to risks, barriers and changes in participant circumstances
- Maintain accurate case notes, progress reports and participant records in line with NDIS requirements
- Advocate for participant choice, independence and positive outcomes
About You
You will be compassionate, organised and proactive, with strong communication and relationship-building skills. Previous experience in NDIS Support Coordination or the disability sector is highly desirable.
Essential requirements include:
- Understanding of the NDIS and participant-centred practice
- Excellent communication, organisation and documentation skills
- NDIS Worker Screening Check and other relevant checks
- Current driver's licence and access to a reliable vehicle
Relevant qualifications in Disability, Community Services, Social Work, Psychology, Allied Health or a related field are desirable but not essential.

Pay: $90.00 – $100.00 per hour
