Central Coast Local Health District (CCLHD) is seeking a dedicated Inpatient Social Worker for a temporary full-time role until 8th November 2026 at Gosford Hospital, where you will play a vital part in delivering trusted care and improving psychosocial wellbeing for our community.
About the Opportunity
As a Social Worker Level 1/2, you will be an integral member of the Social Work Department, working alongside multidisciplinary teams to deliver high-quality, trauma-informed, person-centred care. Your role is vital in supporting patients, families, and carers as they navigate the emotional, social, and practical impacts of illness, injury, and hospitalisation.
This position plays a key role in supporting safe and timely discharge planning, strengthening psychosocial wellbeing, and improving overall patient outcomes. In this role you will:
- Provide comprehensive psychosocial assessment and deliver evidence-based Social Work interventions, including crisis response, brief counselling, advocacy, case planning, and coordination of care.
- Support patients and families through emotional responses to acute and chronic illness, trauma, grief, loss, and adjustment to hospitalisation.
- Facilitate safe and effective transfer of care by linking patients and families with appropriate community-based services and supports.
- Work collaboratively with multidisciplinary teams to contribute to clinical decision-making, risk management, and discharge planning.
- Maintain accurate clinical documentation and contribute to reporting, quality improvement initiatives, and service development activities.
- Participate in professional supervision, ongoing education, and the CCLHD Social Work professional network to maintain best-practice standards.
- Participate in the Social Work On-Call roster, providing after-hours crisis response support to the Gosford Hospital Emergency Department as required.

About You
We are looking for someone who has:
- A recognised tertiary qualification in Social Work and eligibility for membership of the Australian Association of Social Workers (AASW).
- Strong knowledge of AASW Practice Standards and a commitment to evidence-based, trauma-informed, and culturally responsive practice.
- Experience in psychosocial assessment, crisis intervention, counselling, case management, and service coordination within a clinical setting.
- Excellent communication, interpersonal, and teamwork skills, with the ability to work effectively in a multidisciplinary environment.
- A commitment to professional growth, reflective practice, and delivering high-quality, patient-centred care within NSW Health values.
