Santa Clara Home Care is a registered Support at Home provider helping older Australians stay safe, independent and well in their own homes. We deliver in-home care, care management, restorative support and allied health across Greater Sydney. Our approach is relationship-based, clinically grounded, and built around dignity, choice, safety and quality of life.
The role
We’re building our in-house allied health offering and want an Accredited Exercise Physiologist to help shape it. This is hands-on, person-centred community work: assessing function and falls risk, and prescribing and progressing exercise to help older people rebuild strength, balance, confidence and independence at home, including goal-based programs under the Support at Home Restorative Care Pathway, working as part of a multidisciplinary team. The role also includes work as a care partner under Support at Home, includes reviewing exercise, function, mobility and reablement goals that flow naturally from your sessions. The role suits an EP who likes autonomous, reablement-focused work and wants a care team behind them rather than working in isolation.
What you’ll do
Exercise physiology
- In-home functional and falls-risk assessment, and pre-exercise screening, for older people.
- Prescribe, deliver, progress and evaluate individualised exercise programs for strength, balance, mobility, deconditioning and chronic-condition management.
- Goal-based restorative and reablement programs, including under the Restorative Care Pathway, as part of a multidisciplinary team.
- Education and coaching for clients, families, carers and support workers to build confidence and self-management.
- Clear notes and progress records; prompt escalation where medical or nursing review is needed.
Care partner duties
- Reviewing exercise, function, mobility and reablement goals against the client’s plan.
- Identifying changing needs or risks in your area and escalating them appropriately.
- Wellness- and reablement-focused discussions and education; accurate care notes.
- Clinical review of complex needs (medication, wounds, nursing, etc.) sits with our nursing team, not this role.
Exercise physiology is clinical care, delivered under the participant’s separately-funded clinical-care stream. Care-management activity is funded from the capped 10% care-management allocation in each participant’s quarterly budget. The two are documented separately and the same time is never claimed against both.
What you’ll bring
Essential
- Tertiary qualification in exercise physiology and current accreditation as an Accredited Exercise Physiologist with ESSA.
- Current professional indemnity insurance covering your full scope of practice.
- Current First Aid and CPR in line with ESSA’s requirements.
- A police certificate issued within the last 3 years (no precluding offences) or a current NDIS Worker Screening clearance or the ability to obtain one before you start.
- Current driver licence and a reliable, comprehensively insured car.
- A smartphone or tablet for mobile documentation, and confident independent practice.
- A genuine commitment to dignity, choice, privacy and safe care.
Highly regarded
- Experience with older people in community, home-based or aged care settings.
- Falls prevention, balance and strength training, reablement and restorative care.
- Chronic-condition management (cardiovascular, metabolic, neurological, musculoskeletal).
- Familiarity with Support at Home, the Restorative Care Pathway, Home Care Packages or community aged care.
- Confident communication with families, care managers, nurses, GPs, support workers and allied health colleagues.

Who we’re looking for
An EP who is confident and safe working on their own in the community, screens well before prescribing exercise to frail or higher-risk clients, communicates clearly with older people and their families, keeps clean and timely notes, and knows when to escalate rather than push on. If you like goal-based work and helping people stay strong, mobile and independent at home, this will suit you.
Pay and conditions
$66–$80 per hour for ordinary hours, Monday to Friday within the daytime span, inclusive of the 25% casual loading, plus superannuation at the statutory rate (currently 12%). Your classification and pay point are set in writing under the Health Professionals and Support Services Award 2020 (MA000027), Health Professional stream, based on your qualifications, experience and actual duties. Your pay will never be less than the applicable award minimum for your classification, including annual award increases.
Your contract and payslips separately identify the base hourly rate and the 25% casual loading. Weekend, public-holiday, shiftwork and overtime hours are paid at the applicable award penalty or overtime rates and are not absorbed into the ordinary-hours range. Casual engagements have a minimum 3-hour payment period unless a greater entitlement applies.
Travel between clients within an engagement is paid as time worked, and authorised use of your own vehicle is reimbursed at no less than the award motor vehicle allowance (currently about $0.99 per kilometre). Paid time also includes documentation, required meetings, training, supervision and care-plan input.
