About the role
We are looking for a registered nurse with aged care experience who enjoys monitoring and managing aged care information to meet the needs of our residents and our regulatory obligations . The role of the care support coordinator will work within our team to provide support and direction in the areas of compliance and improvement initiatives across the Residential Aged Care and Transition Care Program services.
- Coordinate aged care funding systems and assessments, monitor resident changes and care minutes, validate funding outcomes, and support accurate clinical documentation to maximise funding.
- Coordinate clinical and non-clinical audits, QIP data collection, care evaluations and mandatory training compliance, identifying risks and improvement opportunities.
- Monitor and report on restrictive practices, NDIS requirements, antimicrobial use and aged care regulatory obligations, maintaining registers, documentation and required reporting.
- Ensure accurate and compliant patient information management, support OHS requirements, maintain regulatory and organisational standards, and contribute to quality improvement and special projects.
- Registered Nurse with Australian Health Practitioners Regulation Agency (AHPRA)
- Intermediate computer literacy and skill, including the proficient use of the Internet, Word, Excel where relevant
- Experience in Aged Care
- Excellent record keeping standards
- Current Drivers Licence
- Valid NDIS Worker Screening Check Clearance
- Not subject to an Aged Care Banning Order
