About the Role
We’re currently seeking compassionate and reliable Disability Support Workers to join our growing team in Geelong. You’ll be working directly with NDIS participants to support them in their daily lives — helping them pursue their goals, build capacity, and stay connected to their communities.

Key Responsibilities:
As a Disability Support Worker with Wise Healthcare, you will play a vital role in supporting participants to live safely, independently, and with dignity. Your responsibilities will include:
- Providing direct, person-centred support in the home or community, tailored to each participant’s NDIS goals
- Assisting with daily living activities, including personal care, hygiene, meal preparation, medication reminders, and household tasks
- Promoting social participation by supporting clients to engage in recreational, cultural, or community-based activities
- Encouraging independence and choice while supporting life skills development and confidence-building
- Collaborating with families, caregivers, and allied health professionals to ensure holistic and consistent care
- Following individual support plans, health protocols, and behaviour support strategies where applicable
- Documenting and maintaining accurate progress notes, incident reports, and changes in participant needs or behaviours
- Upholding Wise Healthcare’s core values of respect, compassion, and professionalism in all interactions
Essential Requirements:
- NDIS Worker Screening Check
- National Police Check
- Working With Children Check (Employee)
- First Aid & CPR Certification
- Completion of NDIS Worker Orientation Module
- COVID-19 Vaccination Certificate
- Right to work in Australia
- Driver’s licence and reliable vehicle preferred
Desirable Requirements:
- Previous experience in disability support or a related field is highly regarded
- Certificate III or IV in Disability, Individual Support, Aged Care, or a relevant qualification is a plus
- Ability to work independently as well as collaboratively within a team
- Previous experience working with NDIS participants or allied health teams
- Certificate III or IV in Disability, Aged Care, or Individual Support
- Mental health, autism, or behaviour support experience
- Manual handling or medication administration training
