About The Royal Melbourne Hospital (RMH)
Join one of Victoria's leading public health services and make a meaningful impact as part of our innovative Home First program. This role offers the opportunity to deliver high-quality Social Work services within a well-supported, multidisciplinary team dedicated to providing person-centred care in the community.
As a valued member of the Community Therapy Service (CTS), you will work alongside experienced clinicians while contributing to the supervision and education of social work students and junior staff. You will be supported by an experienced leadership team, including the Social Work Manager, Grade 4 Clinical Lead, and Grade 3 Senior Clinician.
The RMH Social Work Department comprises more than 40 staff working across the City and Royal Park campuses, delivering services across acute, sub-acute, and community settings in a wide range of clinical specialties.
About the Role
In this role, you will:
Deliver high-quality, client-centred Social Work services to consumers participating in the Community Therapy Service (CTS), undertaking comprehensive psychosocial assessments and developing targeted intervention plans.
Provide specialised Social Work interventions, including risk assessment, care coordination, advocacy, counselling, and family and group work.
Collaborate effectively within a multidisciplinary team to achieve positive outcomes for consumers and their families.
Contribute to the ongoing development of the CTS through quality improvement initiatives, teaching, research, and policy or program development.
Support workforce development by supervising junior Social Workers, students, and Allied Health Assistants, while contributing to the growth and excellence of the Social Work profession.
Receive ongoing professional support, supervision, and development from the Social Work Manager, Grade 4 Clinical Lead, and Grade 3 Senior Clinician.
About you
Qualified and experienced Social Worker (either current member or current eligibility for AASW) with demonstrated clinical skills in the sub-acute hospital setting and/or community rehabiliation
An adaptable quick learner with proven ability to work autonomously
Effectively deliver excellent consumer focussed clinical care to patients and their carers
Experience and skills to mentor and supervise junior staff
