We are seeking a Service Coordinator to play a key role in coordinating participant supports, working closely with families and support workers, and ensuring high-quality service delivery.
This role is ideal for someone passionate about person-centred care, strong relationships, and coordinating services that improve participant outcomes.
About the Role
As a Service Coordinator, you will act as the primary link between BET Care Group, participants, families, and external providers.
You will coordinate services, develop support plans, oversee support teams, and ensure supports are delivered in line with NDIS standards and participant goals.
Key Responsibilities
- Coordinate supports in line with NDIS plans and participant goals
- Develop and review Support Plans and Participant Profiles
- Build strong relationships with participants, families, and stakeholders
- Liaise with NDIA, allied health providers, and community organisations
- Supervise and support Disability Support Workers and house teams
- Monitor participant satisfaction and service quality
- Maintain accurate documentation and reports in internal systems
- Manage incidents, complaints, and compliance requirements
- Conduct house checks and ensure WHS standards are maintained
- Participate in rostered on-call duties
About You
You are passionate about supporting people with disability and enjoy working in a role that combines participant engagement, coordination, and team support.
You will bring:
- Experience working in the NDIS or disability services sector
- Strong communication and stakeholder engagement skills
- Excellent organisational and documentation skills
- Ability to manage multiple participants and service coordination tasks
- A person-centred and solution-focused approach
Essential Requirements
- Qualification in Disability, Social Work, Community Services, Health or similar (or relevant experience)
- NDIS Worker Screening Check
- Working With Children Check
- Valid Driver's Licence
- Right to work in Australia
- Willingness to travel between service locations
