Registered Nurse – Community / Disability Care
Able Koala Care Services is seeking a compassionate, reliable and patient Registered Nurse (RN) to join our team and provide high-quality nursing care to NDIS participants in their own homes and within the community.
In this role, you will provide clinical care, health monitoring, education and practical support to participants who may be living with physical disabilities, mental health conditions, complex health needs or age-related conditions.
Our goal is to support each participant to maintain their health, independence and quality of life while working collaboratively with families, support workers and other healthcare professionals.
Hours
Approximately 5 hours per week on weekdays, with the possibility of additional hours depending on participant needs.
Key Responsibilities
- Wound assessment, wound care and dressing changes
- Catheter care and catheter-related clinical assessment
- Monitoring and documenting changes in participants’ health conditions
- Developing and reviewing nursing care plans where appropriate
- Providing clinical guidance and education to participants, families and support workers
- Maintaining accurate nursing progress notes, clinical records and relevant documentation
- Recognising changes or deterioration in a participant’s condition and escalating concerns appropriately
- Supporting participants with complex physical and/or mental health needs
- Working collaboratively with GPs, specialists, allied health professionals, Support Coordinators and other members of the participant’s care team
- Maintaining a person-centred, respectful and professional approach at all times
Skills and Experience
- Experience and demonstrated competence in Wound Care
- Experience and demonstrated competence in Urinary Catheter Care
- Experience working within Community Nursing, Aged Care or Mental Health is highly regarded
- Strong clinical assessment and documentation skills
- Excellent communication and interpersonal skills
- Ability to work independently within the community setting
- Compassionate, respectful and person-centred approach to care
Essential Criteria
- Current AHPRA registration as a Registered Nurse
- Current National Police Check
- NDIS Worker Screening Check
- Current First Aid and CPR certification
- Current Australian Driver Licence
- Willingness to travel to participants’ homes
- Willingness to provide personal care when clinically required
- Previous experience in Community Care, Mental Health or Aged Care
- Appropriate vaccination requirements in accordance with current workplace and public health requirements
