About RMH Mental Health Services
The Royal Melbourne Hospital Mental Health Services provides specialist recovery-focused care for adults and older adults experiencing serious mental illness. Our multidisciplinary teams deliver high quality, trauma informed and person-centred inpatient and community mental health services.
About the Role
The Progression of Care Navigator supports timely discharge planning and consumer flow across the Adult Acute Inpatient Unit. Working closely with multidisciplinary teams and community providers, you will help coordinate safe transitions of care, identify barriers to discharge and support continuity of care for consumers.
About You
You are an experienced mental health nurse with strong organisational skills and a passion for recovery-oriented care.
You bring:
Registration with the Nursing and Midwifery Board of Australia
Postgraduate qualifications in Mental Health Nursing (or equivalent)
Minimum 5 years’ experience in mental health assessment and treatment
Excellent communication and stakeholder engagement skills
Strong understanding of discharge planning and consumer flow within inpatient settings
Commitment to collaborative, consumer-centred care
Strong motivation, highly effective communication skills and passionate about leading other
