Job Description
We are seeking a compassionate and dedicated Social Worker to join our supportive team at Noosa Hospital. In this vital role, you will provide essential psychosocial support to acute inpatients, outpatients, and their families and carers, encompassing both private and public patients.
About the Role:
As a Social Worker at Noosa Hospital, you will play a crucial role in enhancing the well-being and recovery journey of our patients. Your responsibilities will include:

Providing comprehensive psychosocial assessments and interventions for acute inpatients and outpatients.

Offering support and counselling to patients, their families, and carers as they navigate health challenges.

Developing and implementing discharge plans that address the social, emotional, and practical needs of patients.

Connecting patients and families with appropriate community resources and support services.

Collaborating effectively with multidisciplinary teams to ensure holistic patient care.

Advocating for the rights and needs of patients and their families.

Maintaining accurate and timely documentation of patient interactions and care plans.
About You:
You are a qualified Social Worker with experience in a healthcare setting, ideally with acute patients. You possess excellent communication, interpersonal, and advocacy skills, enabling you to build rapport with patients and their families during challenging times. Your ability to work autonomously and as part of a team, along with your commitment to patient-centred care, will be highly valued. A strong understanding of relevant legislation and community resources is essential.
Remuneration
On offer is a base hourly rate ranging from: $41.97 - $62.81 + superannuation based on experience level and qualifications, exclusive of penalties or allowances.
