About us:
Sub-Acute Services (SAS) plays a vital role in supporting patients on their recovery journey by coordinating rehabilitation and geriatric services across Central Queensland. Working closely with specialised inpatient units at Rockhampton Hospital, the team is dedicated to helping individuals regain independence, maximise their quality of life, and achieve their personal health and lifestyle goals.
Our service manages referrals from hospitals, GPs, and health services across the region, while also facilitating a range of outpatient and community-based programs, including Rehabilitation, Geriatric and Amputee Clinics, Day Hospital Therapy Services, and the Interdisciplinary Assessment Service. SAS also supports patient transitions to appropriate care settings, ensuring continuity of care and positive outcomes for patients and their families.
About the role:
Provide advanced clinical leadership and operational oversight for Rehabilitation and Geriatric Services, supporting multidisciplinary teams and driving high-quality patient care.
Lead quality, safety, and risk management initiatives, including clinical governance, service improvement activities, audits, incident reviews, and complaint management.
Manage the day-to-day operations of the service, ensuring the effective use of staffing, financial, and material resources to achieve efficient service delivery.
Act as an expert nursing resource, contributing to clinical decision-making, providing specialist consultation, and promoting evidence-based practice across the service.
Foster a culture of learning, research, and professional development by mentoring staff, identifying education needs, supporting innovation, and leading workforce development initiatives.
About you:
What You'll Bring to the Team:
Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
Strong clinical knowledge and a commitment to delivering high-quality, patient-centred care.
Leadership experience, ideally within aged care, rehabilitation, or other sub-acute healthcare settings.
The ability to lead and support multidisciplinary teams while driving service improvement and clinical excellence.
Postgraduate qualifications in a relevant field are highly regarded and will be viewed favourably.
What we offer:
Total monthly remuneration value of up to $14,447 p.m. is comprised of:
An hourly rate between $70.58 - $76.63 p.h.
12.75% employer superannuation contribution
