NDIS Support Coordinator
Support participants with application, issue, maintenance and process for housing, employment, education, healthcare, and
social programs.
Monitor Allied care services for the participant progress and adjust supports when required.
Advocate for participant rights, choice, independence, and inclusion such any complaints and services
Maintain case notes, reports, and documentation.
Ensure to give correct information about the services according to NDIS guidelines, policies, and safeguarding requirements.
Ensure medical and Allied Services appointments are booked and follow up with the appointments.
Liaise with participants, families, carers, healthcare providers, and community Services with all client’s needs
Coordinate referrals to allied health professionals, counsellors, and support agencies.
Support participants in building daily living and community engagement skills
Pay: $75,000.00 – $90,000.00 per year
