The opportunity
You will play a pivotal role in supporting the delivery of high-quality, person-centred care through Warrigal Home Services by coordinating and managing the care needs of clients living in the community. Working closely with the Care Services Manager, Operations Partner and broader Home Services team, you will ensure services are aligned to individual care plans, client goals and approved support frameworks.
This role requires someone who is highly organised, responsive and confident managing competing priorities in a dynamic environment. You will be responsible for undertaking comprehensive assessments, developing and reviewing care plans, and ensuring services remain responsive to changing client needs. Your ability to balance multiple client caseloads while maintaining a strong focus on outcomes and wellbeing will be essential.
You will collaborate closely with clients, families, service providers, and internal teams including Service Planning and Rostering, to ensure services are delivered safely, efficiently and in line with care plans. Your ability to problem-solve, communicate effectively and respond proactively will be key to minimising service disruptions and supporting positive outcomes for clients.
Through proactive care management, regular monitoring and ongoing engagement, you will support older people to live safely and confidently in their own homes and communities. You will also contribute to continuous improvement by identifying opportunities to enhance care delivery, promote independence and ensure the effective use of client funding to achieve optimal outcomes.
What you’ll bring
Essential:
- Diploma or Certificate IV in Community Services, Aged Care, Case Management or equivalent tertiary qualification.
- Demonstrated experience in care coordination, case management or related roles within aged, health or disability sectors.
- Advanced assessment and care planning skills including the ability to identify client needs, risks and support requirements.
- Experience working in multidisciplinary care environments.
- Strong interpersonal and communication skills with the ability to build relationships with clients, families and service providers.
- Ability to manage a caseload and prioritise competing demands
- Demonstrated understanding of person-centred care and wellness reablement principles.
- Demonstrated ability to network, negotiate and advocate.
- Ability to meet Worker screening requirements
- Valid Driver's Licence.
Desirable:
- Tertiary qualification in nursing, allied health, community services or related
- discipline.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) where applicable
- Experience supporting clients with complex care needs.
- Knowledge of the Aged Care Act and Support at Home reform changes
